The online order forms below will generate sales through Infusionsoft and should be use for all NEW CUSTOMER orders beginning January 1, 2018. Please select the appropriate option for the product(s) and or services(s) the client would like to purchase. In some cases, there is additional information to fill out once the credit card has been capture. Please keep the client on the line until the credit card is processed and all necessary inforamtion captured. For products that require additional information, a follow-up screen will appear after the credit card is successfully authorized. This screen will contain the field for the necessary information to be captured to complete the filing or service.

Required Documentation
For existing entities who are changing to us:
- Copy of the Articles of Incorporaton/Organization
- Change of Registered Agent Form for the appropriate state (Note: in NEVADA there is a $60 fee to change RA)
- Confirm or capture: Name, Physical Address and Contact Information for Custodian of Record (primary communication contact).
- Communication Contact Form and Key Individuals Form (WYOMING only)
For new entities that are not being set up by us:
- We provide either a signature or form accepting as Registered Agent. This is typically provided by the Incorporator.
- Above documentation (for existing entities) must be provided by the Incorporator once the incorporation for the entity has been filed.
